After contacting our office you will receive an email with instructions and registration forms – information provided a head of your appointment will greatly expedite your registration and facilitate a smooth transition from necessary paperwork to your clinical visit with our team.  

Dr. Valentine and Dr. Lim are commited to see you at your scheduled appointment time. If you are not able to make your scheduled appointment time, kindly notify us at least 24h prior your appointment. 

This will allow us to find a better suited time for your needs and provide an opportunity to serve another patient in pain that needs urgent care.

It will also prevent a “late cancelation/no show” charge applied to your account. 

In the event we may be delayed in meeting you during your schudeled time, we will attempt to contact you via your provided contact phone number. We thank you for your consideration when giving us an advance notice, and we appreciate your understanding when we are running behind the expected time.
